The existing treatments involve making lifestyle changes to improve Vulva care and using therapeutic creams. Vulva care includes avoiding soap, powder, perfume, polyester undergarments, tight pants, scratching, shaving and avoiding excessive moisture. Wearing cotton underpants, and no undergarment at night can assist. Therapeutic treatments include using a non-scented barrier cream or applying topical Hormone Replacement Therapy (HRT – vaginal estrogen cream) if not contraindicated.
Patients with strong family history of breast cancer and/or concerns of breast cancer, and patients with current or past history of breast cancer may be advised against using HRT by their breast surgeon.
The success of the existing treatments are greatly affected by how disciplined the patient is in adhering to the application protocols and lifestyle changes. If adherence is good then it can be effective in about 60 – 75% of patients. Deviation from the protocol occurs commonly in elderly patients who have physical limitations in applying the cream, patients who do not like the treatment process or when patients are busy and forget to use the cream as recommended. Once a patient stops applying these treatments ceases the benefits are lost and the symptoms may reappear.
